Bloom's Sliding Fee Scale Policy

At Bloom, we understand that financial hardship can create barriers to accessing treatment. We are committed to exploring various programs to help cover therapy costs for clients who request services. The available programs include:

 

  • I Matter

  • Second Wind Fund 

  • Path4EMS

  • DHS Core Services Program Area 3

  • Colorado Organization for Victims Assistance

  • Therapy Aid Coalition 

  • Special Funding Opportunities through Community Partnership, the Aspen Mine Center, or other
    local funding sources

  • Application for Medicaid and Medicare (Peakcolorado.gov)

Once we have explored these programs and established that they do not cover the costs or clients are unable to apply for any reason, we can discuss sliding scale fees. These fees are determined based on family size, annual gross income, and the Federal Poverty Level Guidelines (FPLG). Family size is defined as the number of people who live together as a household. For example, if you have two adults and one child, your household size is three. If you have three adults and three children, your household size is six. Household income is defined as the total gross income of all members in the household and can include wages, salaries, self-employed earnings and other income. For more information on qualifying household sizes and annual income, please refer to page two of this policy. Bloom’s sliding scale offers five categories of fees based on a percentage of the FPLG, which can be found on page three of this policy. To qualify for the sliding fee scale, Bloom requires proof of income. Acceptable documents include one or more of the following: pay stubs, tax returns, or an employer's letter verifying income and employment. Other forms of income verification, such as Social Security benefit letters, unemployment statements, or evidence of alimony or other support payments, may also be accepted. Please submit these documents to Laura Magnuson at admin@bloomwellnesstherapy.com, along with your completed application. Completed applications and supporting documentation will be reviewed, and eligibility will be determined within three business days of receipt.

Individuals approved for sliding scale benefits will need to reapply in January and July each year. Bloom will send a request for this information to your TherapyNotes account when renewal documentation is due. Furthermore, clients experiencing significant changes in their financial situation (e.g., loss of income or a new job) should notify Bloom and may reapply for a revised discount level. All fees for services are due on a weekly basis, either at the time of service or through Bloom's Electronic Health Records System. While Bloom aims to work with clients on late fees, failure to communicate regarding financial circumstances may lead to interruptions in services or third-party debt collection. Clients are encouraged to stay in close communication with Bloom’s billing specialist and actively work to resolve any outstanding balances to be considered compliant with billing.
